Dr. Butler’s office is pleased to offer a Dental Membership Program to patients not currently covered by dental insurance. We want to help you prioritize your oral health, and this program allows for a complete continuation of preventative care.Our exclusive membership program is a dental savings plan designed to provide greater access to quality dental care at an affordable price. How do I sign up for the Membership Program?
Scroll down and click the Sign Up button underneath the program of interest to you. Fill out and submit the form, and a Madison Family Dental representative will contact you to answer any questions you may have and to get you started with the program. We understand that dental discount plans vary between providers, so please don’t hesitate to contact us with questions.
Do you have other dental discount plans?
Our Dental Membership Program allows for you to receive the preventative care you need to have healthy teeth and gums. While we do offer cash pay discounts this dental discount plan is slightly different. Please call our offices if your questions are not answered after reviewing this page.
Is this an insurance plan?
No, this program is not an insurance plan. Our Dental Membership Program provides our patients with discounted prices on preventative care, as well as a range of common dental procedures. You will pay the discounted rate at the time services are provided.
I am an existing patient, can I join?
All existing patients are invited to join our Dental Membership Program.
What happens if I become insured while a member of the program?
Our Dental Membership Program is for uninsured patients or patients who choose not to use their insurance (for example, if they have out of network insurance). If you become insured, or choose to use your insurance, your membership will be terminated. Dues are non-refundable.
I have dental insurance, can I sign up for this Membership Program?
This offer is not valid in combination with dental insurance of any kind. We sincerely appreciate your loyalty to our practice.

Program Guidelines

The Dental Membership Program is not insurance. The program is offered to individuals and families who do not have insurance, or who choose not to utilize their insurance. Dental Membership Program benefits are not transferable to another dental practice or dental specialty practice. Payment is always due at time of service.

  • Membership is for 1 year beginning on the enrollment date.
  • Membership dues are payable in full upon enrollment and are non-refundable.
  • Cash, check or credit card payments are accepted.
    Membership will automatically renew on the anniversary date unless a written request to cancel is received 30 days prior to the renewal.
  • Payments for additional dental services are the member’s responsibility. Payment is due on the date of service to qualify for the discounts detailed above.
  • Membership must be current to receive the discount.
  • Fees for dental services may change at any time.
    It is the member’s responsibility to schedule and keep all appointments offered as part of the Dental Membership Program.
  • Please notify our office at least 48 hours in advance if you must change a scheduled appointment.
  • No deductibles, no pre-authorizations, no yearly maximums, no waiting period.
  • No exclusions for elective care.

Routine dental care and what it can prevent

  • Gum Disease: Periodontal (gum) disease is a leading cause of tooth loss and may be associated with other chronic diseases, including diabetes and heart disease.
  • Tooth Decay: Also known as cavities, tooth decay is the second most prevalent disease in the United States.
  • Tooth Erosion: Tooth erosion is the loss of tooth structure and is caused by acid attacking the tooth’s enamel.
  • Bad Breath: According to dental studies, nearly 85% of people with persistent bad breath have a dental condition.
  • Mouth Sores: Common mouth sores include canker sores, fever blisters, cold sores, ulcers and thrush.
  • Tooth Sensitivity: Tooth sensitivity means experiencing tooth pain or discomfort from sweets, cold air, or hot / cold drinks.
